How Team League Fee Payment Works:
· The individual to sign their team up for a Comeback sports league will be noted as the Team Captain. The team captain is responsible for timely payment of his/her league fee as well as team attendance to league games.
· In order to reserve your team’s spot in a Comeback league, a $100 deposit must be made.
· The remainder of your team fee is due at the second week of league play (i.e your second game)
· League Fee payment can be submitted by check or by credit card. Checks may be mailed to our office or handed in to a Comeback official.

· PLEASE NOTE: Your team’s spot is NOT confirmed until your team deposit is received.

Individual Sign Up:

In order to reserve a spot in a Comeback league, Individual sign-ups will be asked to select their form of payment during the sign up process. If the individual chooses to pay by credit card then their spot in the league will be confirmed immediately. If the individual chooses to pay by check, their spot in the league will NOT be confirmed until payment is received.

Sign-up and Team Confirmation

Once your sign-up information is received Comeback Sports will send out an email confirming your spot in the league and requesting any additional information that may be needed (such as team name and roster information).

Late Charge

Team Payments that are made past your 2nd scheduled league game are assessed a $50 Late Charge. If full team payment is not received prior to your 3rd scheduled league game your team will be put into "Do Not Play Status".

Bounced Checks

If your check bounces, Comeback will allow a 1-week grace period for you to finalize payment. If payment is not finalized within 1 week then Comeback will be forced to forego your spot in our league. (All NSF and returned check fees charged by the bank must be made payable to Comeback Sports following this Week Period)

Team Cancellations and Refunds Before The Season
A team that has confirmed their spot in our league may cancel their participation for a full refund as long as they notify the Comeback Team at least 7 business days prior to the start of their league. If a team cancels less than 7 business days prior to the start their league, the Captain will forego the $50 deposit and a $50 administrative cancellation fee will be applied. If a team fails to attend the first game of the season without giving prior notice this is viewed as a cancellation and that team is subject to the same $100 cancellation fee. Full Refund: Less (5%) credit card processing fee.

Individual Cancellations and Refunds Before the Season
Individual Sign-ups may cancel their participation in a Comeback sports league for a Refund MINUS $5 PROCESSING FEE if this is done more than 7 business days prior to the start of their league. If the individual cancels their participation less than 7 business days prior to the start of their league then they are subject to a $15 cancellation fee. (This the cost of offsetting refund and transaction costs associated with processing the refund).

Team Cancellations During The Season
Any team that plays a league game will be responsible for the balance of the league fee. No refund will be given. Team cancellations hurt the league and the other teams in it immensely. We will work with your team to apply your team registration to a future league, but cancellation will not render a refund.

Individual Cancellations during the season
Any individual that cancels their participation in a Comeback league after playing in a league game will not be refunded their individual fee. The individual may have their registration applied to a future league, but no refund will be given.

Team No-Show Policy
If a team does not show up to a scheduled game without prior notification of at least 24 hours and a replacement game cannot be found for the opposing team, then the No-show team is subject to a $40 forfeit fee. This fee will be charged to the team captain. Please let us know if you cannot make it! This fee is simply a consolation for the team that showed up and didn’t get to play, because even though they will get a win for your forfeit, Comeback sports is more than just wins and losses, its about the experience!
The required number of players needed in order to avoid a forfeit fee is: 4 for flag football and 5 for kickball. Get a few alternates for your roster to avoid this!

Captain’s Responsibilities
As a team Captain, you are ultimately responsible for your team’s attendance to games and the timely payment of your team’s League Fee. It will be the captain that is charged if your team no shows to a game or if the team’s League Fee is not paid in full in the allowed time period. Although we understand it may not be to any fault of your own, it is the responsibility of the captain and in his/her best interest to keep the team running smoothly. If you don’t think you can count on your teammates to pay their part or simply show up, you may want to consider signing up as an Individual.

Photo Release
I hereby grant [Belleville LLC, DBA COMEBACK SPORTS] permission to use my likeness in
a photograph or other digital reproduction in any and all of its publications, including website entries, without payment or any other consideration. I further give permission to [Belleville LLC, DBA COMEBACK SPORTS] to share this image for use in any and all of its publications, including website entries, without payment or other consideration.
I understand and agree that these materials will become the property of [Belleville LLC, DBA COMEBACK SPORTS] and will not be returned. I hereby irrevocably authorize the [Belleville LLC, DBA COMEBACK SPORTS] to edit, alter, copy, exhibit, publish or distribute this photo for purposes of publicizing the its programs or for any other lawful purpose.
In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph. I hereby hold harmless and release and forever discharge [Belleville LLC, DBA COMEBACK SPORTS] from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
I am 18 years of age and am competent to contract in my own name. I have read this release before signing below and I fully understand the contents, meaning, and impact of this release.
